*Rotateq = Rotavirus vaccine (3-8 months of age only)

*MMR = Measles/Mumps/Rubella

*DaPT = Diptheria/Pertussis/Tetanus

*Hib = Hemophilus influenza b

*Tdap = Tetanus/Diptheria/Pertussis (11years of age and older)

*Menactra = Meningococcal vaccine

*HPV = Human Papilloma Virus (3 dose series completed over 6 months)
